The program was developed by Stanford University’s Patient Education Resource Center and has been successfully implemented throughout the United States and around the world. Nobody wants to have a chronic long term illness. Unfortunately, most of us will experience two or more of these conditions during
our lives. Participating seniors with chronic illness will learn the healthy way to live with a disease. The goal is to achieve the greatest possible physical capability and pleasure from life.
